چکیده

Abstract Beyond the hippocampus, there are no evidence-based dose constraints for eloquent structures which subserve memory and attention/processing speed. We performed a multivariate normal tissue complication probability analysis of post-RT neurocognitive decline, examining dosimetric predictors brain regions in prospective longitudinal clinical trial. 78 patients with primary tumors receiving fractionated RT completed comprehensive evaluation high-resolution volumetric diffusion MRI at baseline 6 months post-RT. Image processing using validated automated segmentation parcellated individual white matter (WM) tracts, cortical regions, hippocampi. Neurocognitive test performance was scored reliable change indices adjusted practice effects: negative scores indicate decline. Univariate logistic regression model building were performed. On univariate mean max to multiple corpus callosum (CC) correlated speed decline including Dmax anterior CC (p=0.011) central (p=0.010), Dmax/Dmean mid-anterior (p=0.006 0.010). Dmean left fornix associated (p=0.023), as increasing age chemotherapy. attention, bootstrapped showed most frequently selected variable CC. Performance nested cross-validation by AUC 0.80 (0.75-0.84); LASSO 0.76 (0.72-0.81) being frequent variable. The top five important variables Random Forest CC, all WM, combined posterior Model 0.66 (0.60-0.71). present first, our knowledge, NTCP speed, beyond hippocampus. Dose several ROIs significantly impairment. These data can guide future cognitive-sparing strategies RT.

A Stochastic Model for the Normal Tissue Complication Probability (NTCP) in Radiation Treatment of Cancer

The normal tissue complication probability (NTCP) is a measure for the estimated side effects of a given radiation treatment schedule. Here we use a stochastic logistic birth death process to define an organ specific and patient specific NTCP. We emphasise an asymptotic simplification which relates the NTCP to the solution of a logistic differential equation. This framework allows for a direct ...

Normal tissue complication probability (NTCP) parameters for breast fibrosis: pooled results from two randomised trials.

INTRODUCTION The dose-volume effect of radiation therapy on breast tissue is poorly understood. We estimate NTCP parameters for breast fibrosis after external beam radiotherapy. MATERIALS AND METHODS We pooled individual patient data of 5856 patients from 2 trials including whole breast irradiation followed with or without a boost. A two-compartment dose volume histogram model was used with b...

A new formula for normal tissue complication probability (NTCP) as a function of equivalent uniform dose (EUD).

To facilitate the use of biological outcome modeling for treatment planning, an exponential function is introduced as a simpler equivalent to the Lyman formula for calculating normal tissue complication probability (NTCP). The single parameter of the exponential function is chosen to reproduce the Lyman calculation to within approximately 0.3%, and thus enable easy conversion of data contained ...

Normal Tissue Complication Probability (NTCP) modeling and validation of quantitative analysis of normal tissue effects in the clinic (QUANTEC) guideline using quality of life questionnaire for parotid gland during head and neck radiotherapy

Introduction: Radiation therapy is the main treatment method for head and neck cancers, which comprise 3–5% of all cancers. A major side effect of this treatment is complication of the parotid glands, i.e. xerostomia, which occurs at relatively low doses. This complication leads to mouth dryness which is the most common problem for head and neck cancer survivors. There are dif...
